EURETINA 2025: Peter Kiraly, PhD, describes how the Young Retina Specialists (YOURS) committee helps surgeons break into the vitreoretinal surgery space

The European Society of Retina Specialists (EURETINA) celebrated its 25th Congress this year in Paris, France. For almost half of its tenure, EURETINA has supported a special program for early-career clinicians. The Young Retina Specialists (YOURS) group provides resources and training for retina doctors under the age of 40. At this year's meeting, the Eye Care Network spoke with Peter Kiraly, PhD, about YOURS opportunities at the meeting, including preparation for one of the European Board of Ophthalmology (EBO) subspecialty exams.

Dr Kiraly is a vitreoretinal fellow at the Manchester Royal Eye Hospital and a clinical research fellow at Oxford Eye Hospital. He also serves within EURETINA as a member of the YOURS committee. This year, he co-chaired the EBO subspecialty exam course on vitreoretinal (VR) surgeries, presenting on vitreomacular interface disorders including lamellar hole, full-thickness macular hole and epiretinal membrane disorders. 

"Retinal specialists should have some vitreoretinal background to be able to deliver excellent care," Dr. Kiraly said in an interview. "The vitreoretinal subspecialty is very competitive and sometimes it's difficult to get hands-on experience." He shared guidance for young retina specialists who want more practical experience in the VR field, encouraging them to seek out opportunities in other countries. "If you feel stuck and can't do VR surgery [where you are], maybe mobility is the key, and you should complete a vitreoretinal fellowship abroad," he advised.

EURETINA 2025: Peter Kiraly, PhD, helps young retinal specialists break into the vitreoretinal space

At the 2025 EURETINA Congress, Prof Korobelnik delivered the Gisbert Richard Lecture, focusing on removal of intra-ocular foreign bodies and minimizing ocular trauma.

Jean-Francois Korobelnik, MD, delivered the Gisbrert Richard Lecture at this year's Congress of the European Society of Retina Specialists (EURETINA). Prof. Korobelnik is affiliated with Hôpital Pellegrin and CHU de Bordeaux, Bordeaux, France. He is also an editorial advisor for 

 His keynote was titled "Removal of intra-ocular foreign bodies: Why, when, how?"

In his lecture, Prof. Korobelnik emphasized the importance of immediate wound closure in the event of ocular trauma. He advocated for delivering antibiotics via intravitreal injections and using advanced vitrectomy machines wherever possible to maximize patient outcomes. He also championed an easy-to-use surgical tool that he believes is underutilized in trauma surgery: intraocular magnets, a key strategy for removing metallic foreign bodies.

"This magnet is really, really a nice tool to elevate the intraocular foreign body from the retina without grasping, without pushing," he said. "Most of the foreign bodies [encountered by surgeons] are metallic, and those magnets are very useful. I was surprised to see in the last few years that some surgeons are not aware of those magnets...I think it's a pity, because at the end, easier surgery is better for the patients."

Prof. Korobelnik also urged clinicians to counsel their patients ahead of surgery to manage expectations following ocular trauma. "It's very important to explain the prognosis," he warned. "Very often patients are young. They are working age. They expect that, with surgery, [their vision] will be back to what it was before the trauma. In some cases, that is the case. But in some other cases, it's more complicated, or not at all better."

The Vit-Buckle Symposium will be a part of the at EURETINA annual meeting in Paris, France again this year. To discuss this symposium, Aleksandra Rachitskaya, MD, FASRS, from Cleveland Clinic Cole Eye Institute and Danilo Iannetta, MD, PhD, from Rome Sapienza University spoke with the Eye Care Network. 

The symposium was designed to showcase challenging surgical cases from presenters worldwide, with a significant portion dedicated to open and honest discussions. Participants believed that learning was most effective when sharing complex medical scenarios rather than just highlighting successes. Iannetta described the session as highly dynamic and interactive, structured around 2-minute surgical video presentations followed by extensive discussions. The format encouraged surgeons to learn from each other's experiences and perspectives. 

The Vit-Buckle Society distinguished itself by welcoming members at all training levels, including medical students, residents, and fellows. Members gained access to extensive educational resources, including webinars in surgical and medical retina, and opportunities to participate in international meetings. 

Iannetta shared his long-standing connection with the society, having first engaged with it in 2014 during a year-long stay in New York. He observed the organization's consistent growth and commitment to creating a multicultural, international scientific community. 

The society will host its annual meeting for April 9-11, 2026, in Las Vegas, Nevada, USA, and is inviting abstracts from participants worldwide. The members emphasized their commitment to supporting young professionals, offering free membership for trainees and opportunities for international participants to present their work. 

Key highlights included the society's presence at multiple international conferences, including recent appearances at the Pan-American meeting and an upcoming event in Asia. Both Rachitskaya and Iannetta stressed the importance of collaboration, knowledge sharing, and supporting emerging medical professionals. The symposium represented more than just a medical conference; it was a platform for global medical professionals to connect, learn, and grow together. By creating an inclusive, interactive environment, the Vit-Buckle Society aimed to advance medical knowledge and support the next generation of eye care specialists.

Ophthalmologist Tarek Hassan introduces revolutionary surgical approach for proliferative vitreoretinopathy, using precise small retinotomies instead of extensive retinal removal to improve patient outcomes.

Innovative Retinotomy Technique Reduces Retinal Surgery Complications, Improves Recovery

Tarek S. Hassan, MD, a professor of ophthalmology at Oakland University, presents a groundbreaking approach to managing proliferative vitreoretinopathy (PVR) through serial relaxing retinotomies. 

"I was given the opportunity to present a series of patients and a concept that we've been working on using serial, relaxing retinotomies in the management of PVR; rather than using a more traditional approach of a circle, linear retinectomy, where we remove several clock hours of retina, make large cuts in the retina," Hassan said.

"Historically, we've thought of relieving tangential traction in cases of PVR as something we have to cut a lot of retina to achieve. And the problem is, our assumption has been that the intrinsic retinal scarring–or foreshortening–that occurs creates lines of traction that go from the periphery to the optic nerve or to attached retina somewhere posteriorly. But in fact, intraretinal scarring, which begins as early as 1 day after retinal detachment, and gets sequentially worse. That intraretinal scarring is fibroblastic glial tissue that proliferates in all directions within the retina, so there are no lines of traction that specifically go from the periphery to the nerve."

Historically, surgeons would remove several clock hours of retina to relieve tension, but Hassan's technique focuses on making small, strategically placed retinotomies. After removing epiretinal and subretinal fibrous tissue, he creates tiny holes the size of a soft tip, which effectively relieve traction in a 360-degree circular pattern. 

"So rather than making large circum linear retinectomies, we've been exploring the idea that if, after you've relieved all epiretinal fibrous tissue cause traction through membrane peeling and any sub retinal proliferation through peeling, then now you're left with the retina that is still foreshortened because of the intrinsic scarring," he said. "Typically, people would make a retinectomy of at least several clock hours, then cut the anterior retina, and then flatten the retina and laser. You're left with a large separation of bare RPE, rather than doing so if there is no further anterior traction–or epiretinal traction–with just foreshortening, we make regularly spaced small retinotomies the size of a soft tip that then relieve the traction, very, very well placed, circular retina retinotomies that relieve traction 360 degrees from the small retinotomy."

This innovative method offers multiple advantages: it removes significantly less retinal tissue (potentially 1/50 or 1/100 of traditional approaches), reduces the risk of re-detachment, prevents long retinal edges from curling, and allows for faster patient recovery. 

By flattening the retina under air and using a scattered laser technique, surgeons can achieve the same anatomical goals with minimal intervention. In the past year, multiple surgeries have been performed using this technique, reporting no recurrent PVR, no hypotony, and improved patient outcomes. 

"when we're talking about stimulating recurrent PBR beyond that operation, certainly cutting less is more when we're trying to achieve the best anatomic outcome," Hassan said.

Ophthalmologist Kapil Mishra discusses radiation retinopathy in uveal melanoma

Ophthalmologist Highlights Vision Challenges in Uveal Melanoma Treatment, Seeks Innovative Solutions for Radiation Retinopathy

Kapil Mishra, MD, assistant professor of ophthalmology at the Gavin Herbert Eye Institute, University of California Irvine, discussed an overview of radiation retinopathy and its challenges in treating uveal melanoma. He presented "Radiation Retinopathy: Future Strategies" during the 2025 

Mishra introduced a unique treatment paradigm in ocular oncology that prioritizes patient survival first, followed by globe preservation, and then vision restoration. 

"When we think about treatment for ocular oncology, and specifically uveal melanoma, there's this paradigm that we follow that we don't typically consider in the world of ophthalmology, because we're so isolated in many ways," he said. "With ocular oncology, it's very similar to the rest of the body in the sense that life trumps all, and then followed by that, there's globe preservation, and then after that, there is vision. So as long as we're able to salvage life and salvage globe, then we can focus on vision."

While treatments for uveal melanoma have significantly improved local tumor control and globe salvation, vision outcomes remain problematic. According to the collaborative ocular melanoma study, approximately 50% of patients undergoing radiation treatment experience vision loss of 20/200 or worse, highlighting a critical area for medical innovation. 

Radiation retinopathy emerges as a particularly challenging disease, characterized by its resistance to treatment and high prevalence among uveal melanoma patients. Mishra emphasized the need to explore both direct and indirect strategies to address this condition. 

The presentation underscored the complexity of ocular oncology, where medical interventions must balance multiple competing priorities. By focusing on life preservation and globe salvation, physicians have made significant strides, but the next frontier involves improving vision outcomes for patients. 

Mishra suggests there is substantial room for developing new treatments and strategies that can mitigate vision loss while maintaining the primary goals of tumor control and patient survival. His research points to potential future developments in ocular oncology that could revolutionize treatment approaches for radiation retinopathy.

Kapil Mishra discusses radiation retinopathy in uveal melanoma

Jennifer Lim, MD, FARVO, FASRS, from the University of Illinois at Chicago, discussed her ARVO presentation on sickle cell retinopathy. She highlighted improvements in retinal surgery techniques and instrumentation.

Joining the Eye Care Network in Salt Lake City, Jennifer Lim, MD, FARVO, FASRS, discussed her Association for Research in Vision and Ophthalmology (ARVO) presentation on sickle cell retinopathy. "We know that sickle cell retinopathy is fraught with complications when proliferative sickle cell stages develop, such as vitreous hemorrhage or retinal detachment," she said. "Unfortunately, when we do surgery for that frequently, we can get complications which include high rates of retinal tears as well as recurrent retinal detachments."

Fortunately, improvements in retinal surgery techniques and instrumentation have increased the fixability of retinal detachments: success rates for these repairs have increased from 50% to nearly all cases.

"We performed a retrospective study where we reviewed all patients with sickle cell retinopathy in the electronic health record, and we found that there were approximately 25 patients of 208 who had surgical indications," she said. "However, we excluded some patients who did not have at least 6 months of follow up or had some missing data. We, therefore, were left with 21 patients, which included 23 eyes."

The vast majority of these patients had either hemoglobin SC or hemoglobin SS subtypes of sickle cell disease. "We found that for patients who had non retinal detachment indications for surgery, that the vast majority of these patients had improvements in vision and had good anatomic outcomes for the patients who had non retinal detachment indications," said Dr. Lim, citing that 90% of patients had significant improvements in visual acuity outcomes. "For the patients who had retinal detachment indications, 13 eyes, the vast majority of these patients also had improved or stable visual acuity outcomes," she added.

Lim also emphasized the importance of early intervention with laser treatment to prevent vision loss and stressed the need for regular screening. To hear more about the study findings, and the clinical implications, watch the full video.

The 2025 Snowmass Retina and Eye Meeting, taking place at the Viceroy Hotel from March 24–28 in Snowmass, Colorado, offers retina specialists an immersive learning experience alongside winter recreation. Course Director Ronald Frenkel, MD, emphasized the meeting’s commitment to innovation in retina, cataract, glaucoma, and the anterior segment. “We wanted to get an update on the latest things and the newest things in retina and in ophthalmology,” he said, underscoring the interactive format designed to optimize engagement.

Retina-focused highlights will include David Sarraf, MD’s Machemer Lecture, which will explore strategies to minimize the risks of pigment epithelial detachment and retinal pigment epithelium tears. Attendees will also gain insights into emerging treatments for macular disease and geographic atrophy. The program will feature discussions on AI’s expanding role in diabetic retinopathy diagnosis and management, fostering interactive dialogue among experts.

Keynote presentations will include the Epstein Lecture on advanced glaucoma by Leon Herndon, MD, and the Ridley Lecture on cataract surgery by Cathleen McCabe, MD.

Maria H. Berrocal spoke with our team about her presentation at the Women in Ophthalmology Summer Symposium being held in Marco Island, Florida.

- This transcript has been edited for clarity.

I'm David Hutton of Ophthalmology Times, Women in Ophthalmology is hosting its Summer Symposium this year in Marco Island, Florida. I'm joined today by Dr Maria Berrocal, who is presenting "Robots and 3D surgery: What is the future and surgical retina?" Thank you so much for joining us today. Tell us about your presentation.

Yeah, thank you, David. Yes, I'm going to be talking about the future of vitra-retinal surgery. In particular, the new advances that have been happening and that are expected to happen in the future. We have 3D heads-up digital surgery, with digital microscopes and manipulation of images. And this has a number of advantages. First of all, it has increased depth of field, up to 3 times. An expanded field of view, increased magnification, it's also ergonomic. But it also allows us to be able to enhance the image. So we can actually see tissues better, and it allows us to perform surgery better. And we already have several of these systems that are out there, the Ngenuity, the Zeiss Artevo, then we also have ways of being able to input OCT and other data into our same image for cataract surgery. It's been done, for example, and we can do it super imposing OCTs on the surgery that we're doing. 

And there's other systems coming out, Bausch & Lomb has a system coming out too, and then we have the bionic system, which is different, because it actually has an exoscope. So what it actually does is, there's a camera viewing the eye, but we wear actually [a] headset, which is very similar to what fighter pilots use when they're navigating. And it allows us to basically move our heads and move the image and control everything with, with our heads. So that is as far as visualization is going. 

As far as robotics, there really, there's nothing really out there quite yet. But what they're working on doing, and there's a number of companies doing different things. It's basically stabilizing our instruments together with eye movements and head movements. So we have a very, very stable surgical environment and at the same time, basically compensating for any tremor that the surgeon may have. And this is very useful for fine details like analyzing vessels inside the retina, which is something very difficult to do, but that can be useful. And there are some companies that will be using AI (Artificial Intelligence) to learn from as we go along to optimize our performance. So it's all very exciting out there if what do we can foresee in the future.

Ultimately, what can this mean for retina specialists in the patients they treat?

Well, I think we're gonna be having better outcomes in our patients because really, you know, everything in surgery is you cannot operate when you cannot see well. The better we actually see tissues, the larger we see them, the better we, we will become as surgeons and the better outcomes our patients will have. And less variables, we can have less tremors, less movements of the surgical field, the better our results will be.

WIO 2023: Robots and 3D surgery: What is the future and surgical retina?

J. Fernando Arevalo, MD, PhD, FACS, FASRS, a member of Modern Retina's editorial advisory board, spoke with our team about clinical characteristics of macular holes that close without surgery at the 2023 ASRS annual meeting.

J. Fernando Arevalo, MD, PhD, FACS, FASRS:

Hello, my name is Fernando Arevalo, and I'm a Professor of Ophthalmology at Johns Hopkins University and Wilmer Eye Institute in Baltimore, Maryland. I'm here at ASRS in Seattle, and 1 of the things I have presented on is clinical characteristics of macular holes that close without surgery. This is a multicenter study, worldwide readiness origins contributed. But, I'd like to acknowledge 1 of our research fellows, Hana Mansour We know that macular holes mainstay of treatment surgery. 

The vitrectomy with ILM peeling and gas with face-down positioning closes over 90% of the macular holes. However, there is a subset of patients that will close without surgery. There are reports that show a small percentage of cases and we wanted to look in this history at the clinical and anatomic characteristics of those full thickness, macular holes that close up spontaneously. We [inaudible] the spectral domain OCT's of those macular holes that closed without surgery from Worldwide International Group. We used an imaging software to account for the differences in different machines of OCT, and 2 masked readers evaluated the OCT's for these macular holes. So, we found 78 patients with 78 full thickness macular holes that closed without surgery. 18 were associated to a blunt trauma, [and] 18 were associated to topical treatment with corticosteroids and non-steroidal, anti-inflammatory drugs. We saw an improvement in visual acuity that was significant. Of the holes that closed, 7 of those reopened, that's about 9% of the holes, and they were closed again, 2 of them with surgery, 1 with topical NSAIDs for refusal [to] any surgical procedure. But from the anatomic point of view, we found a correlation between the height and the diameter of the macular hole in terms of visual acuity improvement. 

Also, closure of the macular hole was associated with the thickness of the macular hole and the size of the macular hole in terms of the basal diameter. In summary, for this study, we feel that if patients have had blown ocular trauma, it is a good idea to give them some time of observation, especially if the patient has a macular hole associated with cystoid macular edema and a macular hole that is smaller than 200 microns. The Modern Retina team caught up with one of our board members, Fernando Arevalo, MD, PhD, FACS, FASRS, who shared information on his presentation on the treatment of progressively symptomatic retinal detachments associated with retinoschisis cases at the 2023 ASRS annual meeting.

Hello, my name is Fernando Arevalo, and I'm a professor of ophthalmology at Johns Hopkins University and Wilmer Eye Institute in Baltimore, Maryland. I'm here in Seattle at the American Society of Retinal Specialists, and 1 of the things that I am presenting is the treatment of progressively symptomatic retinal detachments associated to retinoschisis cases.

I presented on a patient that had, or was treated with phaco-vitrectomy and retinectomy to repair one of those complex detachments. We performed a complete vitrectomy with peripheral vitrectomy as well, the use of perfluorocarbon to flood in the retina, and that was to some traction in the periphery with the phaco to allow us to do more peripheral shaving, and even with that there was still some traction and elevation in the periphery.
In addition, the patient had already a 41 band [in] place, and with this traction is still in the periphery, I decided to perform a retinectomy. Basically the roofing of the inner retina to allow the area of the retina peripherally to flatten, and that allowed the retina to reattach in the laser was applied, perfluorocarbon was injected, and then an [inaudible] change and then silicone oil because it was a monocular patient but gas can also be used.

This technique comes from a study that we published recently on 18 cases with progressively symptomatic retinal detachment associated retinas cases where we usually, most of the cases combination of a buckle and vitrectomy, and we found that, in a follow up of about four years, there was a third of patients that had a failure with 1 surgery, and although their final success rate was 100%, but I think that the fact that the 1 surgery success rate was only 66% emphasizes how difficult it is to repair, these retinal detachments. And more research is needed to evaluate what are the best surgical techniques to repair these kinds of retinal detachments
